Lifeline provides an in home and community support service led by a registered Community Support Nurse with extensive experience of working with older people, people with dementia and their families. This includes home visits, community support visits and day centre provision.
Tailored support is also available for adults living at home and wishing to access additional support.
A full assessment is completed for each family prior to introducing the support package.
Lifeline can prompt and support with tasks including personal care, continence support, medication administration and meal preparation.
Lifeline offers a 'Day Centre at Home' service to support people to participate in activity in the home such as life story and reminiscing, music, baking, gardening.
Lifeline also supports individuals to access their community with outings dependent on the individual's needs and interests. This can include accessing social groups, green spaces, cafe's.
To support older adults, people with dementia and their informal carers to maximise their lives and promote social inclusion, activity and respite.
Lifeline provides a sitter service to enable informal carers to access respite whilst their loved one is supported at home or during community outings.
Carers are able to participate in hobbies, interests and arrange appointments knowing they have regular respite from the caring role.
Lifeline provides a bespoke befriending service to adults and those with dementia.
Befriending offers social interaction, activity and structure to the day, reducing boredom and isolation.
